Intraoperative Decision Making with Rough Set Rules for STN DBS in Parkinson Disease

چکیده
In neurosurgical treatment of the Parkinson Disease (PD) the target is a small (9 x 7 x 4 mm) deep within brain placed structure called SubthalamicNucleus (STN). The goal of the Deep Brain Stimulation (DBS) surgery is the permanent precise placement of the stimulating electrode within target nucleus. As this structure poorly discriminates in CT or MRI it is usually stereotactically located using microelectrode recording. Several microelectrodes are parallelly inserted into the brain and in measured steps they are advanced towards expected location of the nucleus. At each step, from 20 mm above the target, the neuronal activity is recorded. Because STN has a distinct physiology, the signals recorded within it also present specific features. By extracting certain features from recordings provided by the microelectrodes, it is possible to construct a classifier that provides useful discrimination. This discrimination divides the recordings into two classes, i.e. those registered within the STN and those registered outside of it. Using the decision tree based classifiers, the best results have been obtained using the Random Forest method. In this paper we compared the results obtained from the Random Forest to those provided by the classification based upon rules extracted by the rough set approach.

Early outcome of subthalamic nucleus deep brain stimulation (STN-DBS) in advanced parkinson disease in first trial of Iranian patients
Abstract Background: To improve the debilitating features of Parkinson disease (PD) different medical and surgical approaches are available. Subthalamic nucleus deep brain stimulation (STN-DBS) was appeared to be a promising method during last two decades. This study aimed to evaluate early motor outcomes of this procedure in first trial of Iranian patients . Methods: Thirty-seven...
متن کاملAnatomical situation of the subthalamic nucleus (STN) from midcommissural point (MCP) in Parkinson\'s disease patients underwent deep brain stimulation (DBS): an MRI targeting study
Abstract Introduction: It is demonstrated that the degree of clinical improvement in Parkinson's disease (PD) achieved by deep brain stimulation (DBS) is largely dependent on the accuracy of lead placement. In addition, individual variability in the situation of subthalamic nucleus (STN) is responsible for spatial inter-individual fluctuations of the real patient's target. Objecti...
متن کاملRelationship of the P Angle with Stereotactic Arc in Intraoperative Outcome of Patients with Parkinson’s Disease Undergoing Deep Brain Stimulation
Background & Aim: DBS (deep brain stimulation) is a new and successful technique in treatment of symptoms of Parkinsonism especially after drug resistance. Research in this field is mostly designed for evolution of this technique. The present study aimed at evaluating the relationship between the angle formed in midsagittal and STN (sub-thalamic nucleus) axis line and recording l...
متن کاملEthical safety of deep brain stimulation: A study on moral decision-making in Parkinson's disease.
INTRODUCTION The possibility that deep brain stimulation (DBS) in Parkinson's disease (PD) alters patients' decisions and actions, even temporarily, raises important clinical, ethical and legal questions. Abnormal moral decision-making can lead to ethical rules violations. Previous experiments demonstrated the subthalamic (STN) activation during moral decision-making. Here we aim to study wheth...
متن کاملThe effect of bilateral subthalamic nucleus deep brain stimulation (STN-DBS) on the acoustic and prosodic features in patients with Parkinson’s disease: A study protocol for the first trial on Iranian patients
Background: The effect of subthalamic nucleus deep brain stimulation (STN-DBS) on the voice features in Parkinson’s disease (PD) is controversial. No study has evaluated the voice features of PD underwent STN-DBS by the acoustic, perceptual, and patient-based assessments comprehensively. Furthermore, there is no study to investigate prosodic features before and after DBS in PD. The curren...
